Description:

Provides hospice education, advocacy, technical and supportive services. Offers a Hospice Locator tool.

Eligibility:

Hospice Care: Must be certified by two physicians as having a terminal illness and a life expectancy of 6 months or less
Palliative Care: Any individual with a serious illness, regardless of age or stage of disease

Intake Process:

(1) Visit website to use the national hospice locator: http://www.nationalhospicelocator.com/
(2) Call the main number to speak to a staff member about the different programs available and how to qualify and register
(3) Required documentation may include (but not limited to):
a. Physician referrals
b. Health insurance (such as Medicare, Medicaid)

Hours:

Monday to Friday, 7:00 am - 7:00 pm
