Description:
Provides a Clubhouse Day Program for rehabilitation and life skills training for those who have experienced a brain injury or stroke or have a medical condition that affects their brain functioning. Also offers an employment program which includes job coaching, an 8-week work readiness program and supported employment services funded through the Department of Rehabilitation Services (DRS). Provides assistance to veterans in their and/or in the community through the Community Support Services (CSS) program.

Persons with a brain injury or stroke; Be able to transfer with less than 50% help; Must be able to handle their own medications

Intake Procedure
(1) Call office during listed hours to complete a phone intake and to schedule a tour, (2) Meet with the Admissions Coordinator to fill out an admissions packet, (3) A program orientation will be offered on the first day of the program, (4) Participants will be assigned tasks to complete at the agency.
