Description
Long-term and short-term rehabilitative services, including physical therapy, occupational therapy, speech therapy and specialty programs such as fall prevention and treatment of balance issues.
Hours
Admissions: Monday to Friday, 8:00 am to 5:00 pm; Care Center: 24 hours a day
Eligibility
Must be a DuPage County resident for at least 12 months and meet admission criteria; Request for waiver of residency requirement is available.

Intake Procedure
(1) Contact the Admissions Department at 630-784-4315 for details on admissions process and requirements, (2) Pre-admission screening is required for all new residents, (3) Rooms assigned by availability during time of admission, (4) Priority for room preferences given to medical necessities.
