Description:

Provides pediatric and adult care, including physical exams, vaccinations and chronic disease management; mental health care including psychiatry and medication management; women's health care including pre-and post-natal care and cancer screening; and family planning including birth control, free pregnancy testing and STD/HIV testing.

Also offers WIC, insurance and public benefits enrollment.

Eligibility:

Families and children of all ages

Intake Process:

(1) Call 877-692-8686/877-MY-AUNT-M or visit the Patient Portal at https://www.auntmarthas.org/become-a-patient/ to request an appointment
(2) Documentation will be required during appointment, including (but not limited to):
a. Photo ID
b. Insurance Card
c. Current medications
d. Proof of income (if not insured)

Hours:

(1) Monday to Friday: 8:30 am - 4:30 pm
(2) 1st and last Saturday of the month: 8:00 am - 12:30 pm
