b. Social Security Numbers for the applicant and qualified dependents
c. Account numbers for any current health insurance (e.g. Medicare, private insurance, insurance from employer)
(2) Apply by Phone: 1-877-222-8387, Monday to Friday, 8:00 am - 8:00 pm
(3) Apply by Mail:
