Intake Procedure
(1) Visit the website listed above and search for provider within your state of residence. (2) Choose a provider and fill out the referral form. (3) Once the form is completed, a notification will be sent to both the provider and Give an Hour. (4) Expect to receive a response in 5 days from the provider contacted.
