Provides assistance dogs and ongoing support to individuals with disabilities. Provides service, hearing, skilled companion and facility dogs.

(1) People with physical or developmental disabilities who can demonstrate that an assistance dog will enhance their independence or their quality of life
(2) At least 18 years old

Intake Process:

(1) Complete the online application request form or call 1-800-BARK to receive one by mail
(2) An application packet will be mailed to you within 4-6 weeks
(3) Complete and return the application to the training center indicated
(4) A telephone interview may be scheduled
(5) If appropriate, medical and/or professional reference forms must be filled out
(6) A personal interview may be scheduled
(7) The review committee will determine acceptance
