Provides orthodontic services to children of low-income families.

Primary Services:


Children of low-income families must:
(1) Be between the ages of 8 and 18 (depending on the state of residence) when treatment begins
(2) Not currently be wearing braces
(3) Have a current general dentists and good oral hygiene
(4) Have a total household income at or below 200% of federal poverty level
(5) Have no other resources or means through which they can receive orthodontic treatment

Intake Process:

(1) Complete and electronically sign online application
(2) Mail or fax a copy of the following items to DOS Coordinator:
a. Previous year's tax return for the household or Social Security awards letter
b. Completed dentist referral form to be completed by child's regular dentist:
c. Letter from the child explaining why he/she wants the braces and how he/she intends to care for them
(3) Submitted application will be placed on waiting list
(4) Once application reaches top of waiting list the family will be contacted by the DOS Coordinator and an intake will be completed
(5) Upon acceptance into the program, family will have 14 days to submit non-refundable fee
(6) Once the non-refundable fee is paid, the DOS Coordinator will refer the child to a volunteer orthodontist. Family will then be notified of the orthodontist's name and phone number to schedule an appointment for an examination
(7) The process may take anywhere up to 6 weeks' time
