Intake Process:
Veterans are encouraged to enroll in the VA Health Care System.
(1) Visit www.hines.va.gov/patients/eligibility.asp to complete an "Application for Health Care Benefits"
(2) Submit the form online, mail it to Hines VA Hospital, Eligibility, 5000 South 5th Avenue, Hines, IL 60141, or bring the form to the hospital
(3) Include a copy of both sides of your current insurance card and a copy of your DD214 "Armed Forces Report of Transfer or Discharge"
