Description

Processes applications for door-to-door shared ride service for riders with a disability who are not able to use public transportation with fixed routes.

Hours

Website: 24 hours a day, 7 days per week; Mobility Services Helpline: Monday to Friday, 8:30 am - 5:00 pm.

Eligibility

Must have a disability or a qualifying health condition; Unable to use the Chicago Transit Authority or Pace fixed routes for some or all of their travel.

Intake Procedure

(1) Call the Mobility Services Helpline to request an application (2) Complete the application https://fares.rtachicago.org/ and call the Helpline to schedule an in-person interview (3) Following the interview, eligible riders will receive a certification letter, a guide, and a photo ID card within 21 days.

Applications available in English, Spanish, Braille, or on audiotape; Interpreters available for American Sign Language (ASL) and Spanish.

              Description

              Reduced fare permits to ride with the Chicago Transit Authority (CTA), Metra, and Pace.

              Required Documents

              Valid Photo ID; Wallet-sized color photo of yourself/applicant that is about 2 inches by 2 inches; If you are a person with a disability, proof of the disability (i.e. Social Security Disability payments, Veteran's Administration letter, etc.); If you are receiving Medicare, verification of Medicare status.

              Eligibility

              65 Years or Older or a person with a disability; Cannot be enrolled in the Illinois Department of Aging's Benefit Access program.

              Fees

              No Fees, Fees apply for replacement Permits or for pictures to be take on site.

              (1) To apply in person, call the main number to find a location nearest you. (2) To apply by mail, download either the Senior or Persons with Disabilities application from the website and return the completed application to the address as listed on the application https://fares.rtachicago.org/. (3) Provide required documentation. (4) If approved, a permit will be received by mail in about 3-4 weeks.
