Intake Procedure
(1) Call or fill out online form to make an appointment. (2) During the first phone interview the individual's needs will be assessed before being connected to a therapist. (3) Documents that may be requested during interview includes Identification, Insurance Card. (4) During the first in-person appointment, the therapist will ask questions and have a discussion to identify the issues and concerns of the applicant and proceed to create a treatment plan accordingly
