Monday to Friday, 8:00 am - 4:30 pm; Visitors: Monday to Friday, 11:00 am - 7:00 pm

Required Documents

A copy of both sides of current insurance card (including Medicare or Medicaid); Copy of DD214; For Purple Heart recipients - copy of award letter if 'Purple Heart' is not noted on DD214; social security numbers for self and all qualifying dependents.

Eligibility

Those who served in active military, naval or air service; Former service members separated under any condition other than dishonorable; Current and former members of Reserves or National Guard who were called to active duty and completed the full period for which they were ordered; Veterans who enlisted after September 7, 1980 or entered active duty after October 16, 1981 must have served 24 continuous months or the full period for which they were called to active duty
