Monday to Friday, 9:00 am - 5:00 pm

Description

Training, job placement and job retention support services for individuals with disabilities. Programs include the Inclusive Pathways Program for Chicago Public School students with disabilities, and the Ticket to Work Program that provides direct job placement services.

Required Documents

(1) Proof of disability (verification by approved medical professionals) (2) Proof of Supplemental Security Income or Social Security Disability Insurance (SSDI)

Eligibility

(1) Individuals 18 years of age or older living with a verified disability (2) An open case with the Department of Rehabilitation Services (DRS) may be required.

    Eligibility

    (1) Individuals 18 years of age and older with intellectual/developmental disabilities (2) Does not need medication administered to them during program hours (3) Does not post a safety risk to self or others.

          Description

          Community Integrated Living Arrangements (CILA) with support services services with staff available 24/7. Available to individuals who live on their own or with roommates or family.

          Eligibility

          A primary diagnosis of a severe, chronic disability due to a mental or physical impairment or combination of mental and physical impairment that was diagnosed or clearly detected before the age of 22.

          Fees

          60D CILA funding through the Division of Developmental Disabilities (DDD) under the Illinois Department of Human Services.
