May include, but not limited to: (1) Photo Identification, (2) DD214, (3) Social Security card, (4) Income verification for previous 6 months, (5) Bank statements for previous 6 months.
Eligibility
Veteran or a Veteran family member(s); Meet income requirements; Homeless or at risk of becoming homeless.

Call the Program Manager at 630-871-8387 ext. 608, or email Megan@MSHV.org to complete a phone screening and set up an intake appointment.
