Description:
Provides comprehensive case management, educational support, and employment planning to program participants who are striving to become self-sufficient.
Hours:
Monday to Friday, 8:00 am - 4:30 pm
Intake Process:
(1) Call to complete an intake.
(2) A Case Manager will contact the applicant to complete further assessment and program description by phone.
(3) If appropriate, applicant will be scheduled to attend a program information session
(4) Information session reminder letter will be mailed that includes list of documents to bring that are required to apply to program
(5) Application to program will be completed at information session during which time required documentation and paperwork will also be collected
(6) Applicants are requested to attend a financial literacy workshop prior to or during their participation in the program
(7) Required documents may be collected electronically and meetings can be held in person or remotely through video

(1) Have a projected yearly income at or below the income guidelines for their family size
(2) Live in DuPage County
(3) Be at least 18 years of age
(4) Have at least one child below age 18 living in the household
OR
(1) Hold a Housing Choice Voucher from the DuPage Housing Authority
(2) Reside in an eligible area
(3) Be at least 18 years of age
(4) Be listed as head of household for purposes of income eligibility or rent

Eligibility
(1) Have a projected yearly income at or below the income guidelines for their family size, (2) Live in DuPage County, (3) Be at least 18 years of age, (4) Have at least one child below age 18 living in the household, OR (1) Hold a Housing Choice Voucher from the DuPage Housing Authority, (2) Reside in an eligible area, (3) Be at least 18 years of age, (4) Be listed as head of household for purposes of income eligibility or rent.
