Description
Eye exam and up to one year of follow-up care for any condition diagnosed during initial exam for seniors 65 and older who are eligible.

U.S. citizen or legal resident; Age 65 or older; Not belong to an HMO insurance plan or have eye care benefits through the VA; Not seen an ophthalmologist in 3 or more years
