(2) May include: Recorded Property Deed or Title Insurance Policy; Proof of Age: Driver's License or State I.D. or Passport or Birth Certificate or Baptismal Certificate showing DOB; Exemption application signed by senior citizen and notarized by a notary public. The application is available at our office, the DuPage County Supervisor of Assessments office or their website â www.dupageco.org/SOA; Certification from the Illinois Department of Veterans Affairs (if seeking disabled veterans exemption)
