Description:

Offers HIV testing, counseling, education, support groups, counseling, psychotherapy, advocacy, information and referral, as well as senior services for the gay, lesbian, transgender, queer and bisexual (LGBTQ) community in the Chicago area.

Hours:

(1) Monday to Sunday, 8:00 am - 9:00 pm

Intake Process:

(1) Violent Prevention and Support: Call the LGBTQ Violence Resource Line at 773-871-CARE(2273)

(2) Behavioral and Mental Health Services:
a. Call 773-472-6469 x460 to schedule a no-cost initial screening appointment.
b. Phone interview will include a set of brief questions and schedule best time for initial screening.
c. Initial screening appointment will take around 1 hour, to assess specific needs and the programs best suited to meet them.
d. Initial screening clinician will then work with the rest of the team to match up client/patient with one of the therapists.
e. Specific private health insurance accepted. Sliding-scale fee structure in place, details available on website.

(3) HIV Testing:
a. Call the listed number or text 'Get2Zero" to 797979 for more information
b. To schedule a testing appointment call HIV testing number or text 872-243-1004 for walk-in times. Appointments Preferred.

(4) Senior Program: Call or email with questions and for information on how to sign up.

Silver Fork is a 9 week culinary arts and job readiness program for the gay, lesbian, transgender, queer and bisexual (LGBTQ) community in the Chicago area who want to enter/re-enter the hospitality industry. Curriculum covers kitchen, job readiness, and sanitation.

(1) Tuesday to Thursday, 10:00 am - 3:00 pm; (2) Schedule is subject to change on national holidays, availability of guest chefs, field trips and any other issues; (3) Full attendance is required to graduate from the program
