Hours:
Monday to Thursday, 8:30 am - 5:00 pm
Friday, 8:30 am - 4:00 pm
Intake Process:
(1) Call during listed hours to complete an intake
(2) A staff member will walk you through the different programs offered
(3) If appropriate, an appointment will be scheduled
(4) Documentation required includes but is not limited to:
a. Photo ID
b. Health insurance card

(4) Private Insurance
