Required Documents
Original Social Security cards or ITIN cards/letters of all individuals to appear on return; Photo ID; Birth dates for all individuals to appear on the return; Bank account information, if applicable; Income statements for tax year; Expense information for tax year; Proof of health insurance coverage.

Meet income guidelines; Have all required documents present; Married taxpayers filing jointly must both be present on site.

Intake Procedure
(1) Call 312-588-6900 to schedule an appointment, or schedule online. (2) Pick up Intake Packet at your local site and fill out before appointment if possible. (3) During appointment, volunteer staff will help answer questions and scan tax documents.
