Free care coordination for children with special healthcare needs and their families. Coordination services may include: developing care plans, accessing diagnostic tests, finding specialized medical are, helping families understand and maximize their coverage benefits, liaising with schools and doctors, transportation for medical appointments, adulthood transition, locating community funding opportunities.

Children who reside in Illinois and living with chronic medical conditions including, but not limited to, cerebral palsy, epilepsy, hearing loss, blood disorders, cardiovascular impairments, eye impairments, pulmonary and orthopedic impairments, and others.
