State program offering an alternative to nursing home care for older, low-income residents and those with physical disabilities covered by Medicaid. Offers web listings of all operational Supportive Living Program (SLP) providers in Illinois, who offer elderly (65 and older) or persons with physical disabilities (22 - 64 years old) personal care and other services (depending on the population served by the facility) which allow residents to live independently and take part in decision-making.

Monday to Friday, 8:30 am - 4:30 pm

Intake Process:

(1) Locate a supportive living program (SLP) using the online directory (2) Call the SLP to get help with arranging all required assessments

Eligibility:

(1) Persons with physical disabilities (as determined by Social Security Administration) between 22-64 years of age
(2) Persons 65 years of age or older
(3) Resident of Illinois
(4) Citizen or legal alien resident of the U.S.
(5) Has had a tuberculosis test that indicates the absence of active tuberculosis
(6) Be checked against required sex offender websites
(7) Cannot be participating in any other home and community-based services waiver
(8) Has income no less than the current maximum allowable amount of Supplemental Security Income (SSI)
