Also offers WIC, insurance and public benefits enrollment.

Monday, 8:30 am - 6:00 pm
Tuesday & Thursday, 8:00 am - 5:00 pm
Wednesday, 8:00 am - 5:30 pm
Friday, 8:00 am - 5:30 pm
Saturday, 8:00 am - 12:00 pm (except 4th or 5th Saturday of the month)

Intake Process:

(1) Call 877-692-8686/877-MY-AUNT-M or visit the Patient Portal at https://www.auntmarthas.org/become-a-patient/ to request an appointment
(2) Documentation will be required during appointment, including (but not limited to):
a. Past medical history
b. Current medications
c. Pharmacy information
d. Photo ID
e. Insurance card
f. Immunization records (if available)
g. Proof of guardianship
h. Contact information for your DCFS worker (if applicable)
i. Proof of income (if currently uninsured)
