Contact and demographic information; Health insurance information; Diagnosis and current medication; Prescribing physician's name and phone number; Estimated household income and household size
Eligibility
Must have a valid Social Security number; Meet Income guidelines; Must have insurance coverage that covers at least 50% of treatment expenses; Diagnosed with a disease that is covered and the program must be accepting enrollments; The prescribed medication patient is seeking assistance for must be FDA-approved to treat the covered diagnosis
