Owner of subject property; Taxpayer (who has a financial interest in the payment of taxes), dissatisfied with the decision of the Board of Review; Illinois licensed attorney representing the owner, taxpayer or taxing body; Taxing body that has a tax revenue interest in the decision

Intake Procedure
(1) A petition must be filed within 30 days of written decision (2) 1 copy of completed appeal form must be submitted to PTAB's Springfield Office along with: (a) 1 copy of your evidence (b) 1 copy of Board of Review's original final decision letter (3) 3 copies of the appeal form and evidence needs to be submitted if the submission is over 500 pages in length
