Required Documents
Documentation of confirmed appointments from patient's treating medical professional, such as an MD, DO, or PA-C; Current photograph of patient; birth certificates of patients under 18.
Eligibility
Children in need of specialized, out-of-state medical care; Parents/legal guardians travelling with the child.

No fee for child patients, Self-pay for adult travelers
Intake Procedure
(1) Complete online application (including requested documentation) and submit by email, fax or mail. (2) Both financial and medical documentation needed. (3) Applications should be submitted at least 14 days prior to departure date.
