Eligibility
Individuals who have determined they are a problem or compulsive gambler and can no longer gamble responsibly, including those affected by video game gambling and sports wagering.

Intake Procedure
(1) Choose a designated gambling treatment provider location from list on website (2) Call to schedule an appointment (3) Call 877-814-8832 for non-English speakers (4) Bring valid photo ID (5) Enrollment process takes between 30 to 45 minutes
