(1) Resident of New York or New Jersey, (2) Available to cancer patients, their caregivers, anyone experiencing grief from cancer-related loss, or children 5 years of age and over.

Persons who have a diagnosis of cancer confirmed by an oncology health care provider; Being actively treated for cancer; Live in the United States; Income/Federal Poverty Limits apply

(1) Call 800-813-HOPE to speak with a social worker and complete an initial interview (2) Those who are approved as eligible based on the initial interview will be mailed/emailed an application and required documentation will be requested (3) Application must be completed and submitted following the instructions on the website.
