Hours:

Monday to Friday, 6:00 am - 7:00 pm

Intake Process:

(1) Call 630-294-6721 or visit website for start process of signing up for ride service
(2) Need to submit the following:
a. Income Verification
b. Copy of Photo ID
c. Proof of Residency
d. Copy of Medical/Health Insurance Card
(3) Application is sent after initial phone screening is complete
(4) Fees charged will depend on service area client is travelling to and from
