(1) Central Intake: Monday to Friday, 7:00 am - 7:00 pm
(2) Outpatient Services: Monday to Friday, 7:00 am - 7:00 pm
(3) Residential Services: 24 hours a day, 7 days a week

(2) Information requested during intake will include (but not limited to) name, address, date of birth
(3) Documentation required for residential admission:
