Description:

Veterans Administration (VA) travel reimbursement and assistance program. Various forms of transportation available for veterans travelling to and from the VA for health care purposes. Also provides shuttle bus service from Aurora and Hoffman Estates VA offices to Edward Hines, Jr. VA Hospital.

Hours:

(1) Monday to Friday, 8:00 am - 4:30 pm
(2) Transportation Services: 7 days a week, 24 hours a day
(3) Claims Filing Helpline: Monday to Friday, 7:00 am - 7:00 pm

Intake Process:

(1) How to request travel reimbursement:
a. Visit the Beneficiary Travel Office after completing an appointment, or
b. Call the office and complete the VA Travel Reimbursement Form 10-3542, or
c. Fax or mail the completed and signed Form 10-3542 to the local Beneficiary Travel Office
d. Submit a claim on website after setting up an online account: https://www.va.gov/health-care/get-reimbursed-for-travel-pay/
(2) Applications for reimbursement/payment must be submitted within 30 calendar days from the date of completed travel or date the applicant was notified for travel payments
(3) Application forms available onsite and from website
(4) Call to verify eligibility for having the deductible waived
(5) Call or visit website for most current reimbursement rates
(6) Call the main number and Dial "0" for interpretation services if needed

(2) Traveling for treatment of a SC condition, or
(3) Receive a VA pension, or income does not exceed maximum annual VA pension rate
(4) Traveling for a scheduled compensation or pension
(5) Medical condition requires an ambulance or a specially equipped van as determined by a VA clinician, and
(6) The travel is pre-authorized (authorization is not required for emergencies if a delay would be hazardous to life or death)
