Intake Procedure
(1) Apply online or download the application form from website (2) Mail-in applications must be sent to the address specified on the form, or emailed as an attachment (3) All supporting documentation must be provided within 30 days of application (4) After the applicant's eligibility is confirmed, the Board of Directors will determine the amount of assistance the applicant is to receive
