Intake Process:

(1) Go to website and select 'Request Assistance'
(2) Select what type of assistance required and complete required screening:
a. Commercial Airline Ticket
b. Ground Transportation
(3) A minimum of 21 days' notice is required before travel
(4) Provide required documentation

Eligibility:

(1) Patients must be ambulatory (able to walk independently, enter and exit the plane with little or no assistance) and require no medical care (from doctor, nurse, etc.) en route to destination
(2) If patient requires oxygen, unless provided by the airlines, patient must have a portable oxygen concentrators (POC) approved by the FAA or the airlines
a. Patient is responsible for making these arrangements and paying for travel fees associated with the oxygen
(3) Meet income guidelines
