(1) Call 630-407-6500 to complete a phone intake and register during the hours of 8:00 am - 4:30 pm, Monday to Friday (2) If eligible, an appointment will be made to collect required information and documentation (3) Once registered, call 800-713-7445 during the listed 'Scheduling' hours to schedule a ride at least 24 hours before appointment time.
