Description
Counseling services for physically, emotionally and/or economically vulnerable older adults, including family and individual counseling, case management, and resource referrals. Includes in-home or office assessments, family and individual counseling and support groups.
Hours
Monday to Friday, 9:00 am - 5:00 pm
Eligibility
Persons 65 years and older and families; Grandparents raising grandchildren

Sliding scale, Medicaid, Medicare, Private insurance, Self-pay
Intake Procedure
(1) Call to complete a phone intake, and to discuss the reason and services requested. (2) If appropriate, an appointment will be scheduled.
