(2) Insurance/medical card
(3) Method of payment
(4) List of current medications (if applicable)
(5) Proof of guardianship/power of attorney (if applicable)
(6) Related dental records / Current radiographs (if applicable)
Program Fees:
Clinics operated by student dentists operate on a reduced fee, and accept:
(1) Cash, personal checks, money orders, credit cards and Debit Card accepted
(2) Medicaid (public aid) cards accepted only for covered Medicaid (public aid) services; not accepted for faculty practice
